Therapeutic Communication: The One Skill That Appears on Every NCLEX Domain

Therapeutic communication is not a soft skill — it is a clinical tool. The NCLEX-PN tests it in every content domain because the patient's words are clinical cues. What a patient says, how they say it, and what they avoid saying are all data points for the LPN to collect, document, and report. Getting therapeutic communication wrong means missing clinical information and providing substandard care.

The Non-Therapeutic Responses to Eliminate First

Non-Therapeutic TypeExampleWhy It Fails
False reassurance"Don't worry, everything will be fine."Shuts down communication; dismisses patient's real concern
Giving advice"If I were you, I would..."Removes patient autonomy; not within nursing scope
Changing the subjectPatient expresses fear; nurse immediately talks about the procedureSignals the patient's feelings don't matter
Closed questions"Are you in pain?" (Yes/No only)Limits data collection; misses context
Offering opinion"I think you should accept the surgery."Influences autonomy; outside nursing scope

The Therapeutic Techniques That Work

TechniqueExampleWhy It Works
Open-ended question"Tell me more about what you are experiencing."Invites full data collection; patient-led
Reflection"It sounds like you are worried about going home."Demonstrates active listening; confirms understanding
Silence[Pausing to allow patient to process and continue]Creates space for deeper sharing
Clarification"When you say it hurts, can you describe what the pain feels like?"Sharpens clinical data; reduces assumptions
Offering presence"I am here with you right now."Addresses psychosocial need without false promises
